1. What is the exact address of where in NY this is taking place? Do you know the person’s name that is committing the neglect or abuse?
2. What County/Town are you calling/emailing from? What is your email/telephone number?
3. What type of animals do you believe are being abused/neglected and how many?
4. Do you have personal knowledge of the situation? If not, who does? Do you have that person’s contact information?
5. Did you file a report with any police agency or SPCA yet, and if so, what agency was it and what was the outcome of the call?
6. Please make sure we have contact information for you.
